One of the most common areas for physicians and other providers to have
compliance exposure with documentation and coding is with evaluation &
management (E/M) services. Assigning correct codes that are appropriate
for what the provider actually documented is commonly misunderstood.
In our work as auditors for numerous private practice and hospital
employed physicians, rates of accuracy are often less than 50% for E/M
services. This session will provide a breakdown of the components that
must be documented to support an E/M services and the requirements
within those components for each level of service.
We will discuss proper Documentation of:
- Chief Complaint; - History of Present Illness; - Review of Systems;
- Past Family Social History; - Assessment and Plan
