Please review the registration types and fees outlined below before registering for the event.
EBAA Cancellation Policy:
All requests must be submitted in writing. Cancellations received on or before October 1 will result in a refund minus a 25% administrative fee. Cancellations received after October 1 but before November 1 will result in a refund of 50% of the registration fee. No refunds will be given after November 1.
To register yourself, click the self-registration button below. To register another person (or multiple people) from your organization, select the group registration. When you see a list of individuals that are linked to your organization, you can select the individuals you would like to register. If there is a physician that you need to register under the physician rate, please select "change rate" in the rate column and select the appropriate rate.
Please do not register another person for the event under your personal EBAA account. If you would like to register another individual and they are not linked to your eye bank or would like to mail a check, please use this form. If you have questions about registering other individuals, please contact Genevieve Casaceli prior to registering them.
If you are an EBAA Member and not yet in the EBAA database, please contact Genevieve Casaceli to be set up in the database.
Cornea Society Members can register by logging in with the account they have used to register for previous Cornea and Eye Banking Forum events, or by creating a new account and then registering.
