Thank you for your interest in attending the 2020 EBAA Virtual Annual Meeting.
Please review the registration types and fees outlined below before registering for the event.
Eye Banker Registration: Includes individual access to all live sessions and on-demand programming. Required to earn Continuing Education Units (CEUs).
Physician Registration: Includes individual access for physician registrants to all live sessions and on-demand programming, and eligibility to earn CME for select sessions. Intended for corneal surgeons, Medical Directors, Residents, and Cornea Fellows. Note: If a Medical Director needs to attend the Medical Advisory Board (MAB) or Medical Directors Symposium for the purposes of the eye bank’s accreditation status, the Medical Director needs to register as a physician registration.
Eye Bank Group Pass: The group pass allows a specific number of staff to attend the live events and an unlimited number of staff to access the sessions on-demand. Registration categories are based upon domestic tissues placed in 2019. Note: CEUs or CME cannot be earned through the group pass.
EBAA Cancellation Policy:
All requests must be submitted in writing. Cancellations received on or before May 29 will result in a refund minus a 25% administrative fee. Cancellations received after May 29 but before June 12 will result in a refund of 50% of the registration fee. No refunds will be given after June 12.
To register yourself, click the self-registration button below. To register another person (or multiple people) from your organization, select the group registration. When you see a list of individuals that are linked to your organization, you can select the individuals you would like to register. If there is a physician that you need to register under the physician rate, please select "change rate" in the rate column and select the appropriate rate. The eye bank group pass can be registered under any staff member's account. Please do not select the individuals' names who will be attending through the group pass during registration. After registration for the group pass is complete, the confirmation email will include a link where your organization can submit the names of people who will need access to the Annual Meeting.
Please do not register another person for the event under your personal EBAA account. If you would like to register another individual and they are not linked to your eye bank or would like to mail a check, please use this form. If you would like to use a paper form to register multiple staff, please use this paper form.
