SKILLED DOCUMENTATION FOR NURSES:

When the the priority of therapy for skilled services goes away proposed PDPM (patient driven payment model), Oct 1, 2019, will your daily nurse notes support skilled nursing care?

Per the RAI; documentation must substantiate the residents need for Part A services. The RAI also states; completion of the RAI / MDS does NOT remove the NH responsibility to document a more detailed assessment of issues relevant to the resident.

Will your daily nurse documentation support the SKILLED services provided and hold up in a Medicare review? Will your current nurse's notes for skilled care identify and support the complexity of skilled nursing care provided?

In the past, therapy notes have been under a great deal of pressure to support the skilled services provided. Going into PDPM the switch is to Skilled Nursing. Will your nurse's notes support the residents need for “daily” nursing services?

In this class, Colleen will guide you through the Medicare skilled care criteria and expectation of supportive documentation.

Current nursing documentation examples will be reviewed.
