Panelists from each stop on that journey will illustrate how interoperability improves patient care through care coordination and automation and consider whether information blocking speeds that journey. Panelists will also discuss how the federal government’s interoperability rules are driving exchange of health data across care settings/provider types (e.g., pharmacies, physicians, hospitals, etc.) and how various elements of care and services needed in long-term and post-acute care interface with the electronic medical record. How the Trusted Exchange Framework & Common Agreement (TEFCA) will affect health exchange in the long term and post-acute care (LTPAC) sector in 2022 will also be discussed.
