DESCRIPTION:
EHRs have the potential to improve healthcare through health information exchange, care coordination, and research. However, issues in data integrity pose a significant challenge to using the information for these purposes. Particularly, patient identification matching has been identified has as a major problem contributing to data integrity issues. Deaths due to medical errors are the third leading cause of death in the U.S., killing about 400,000 patients every year and more than 1,000 people each day. Of the 400,000 deaths per year, approximately 195,000 occur because of identity errors. Despite best practices in the area of patient access and medical record management to avoid duplicating patient records, including scrubbing the master patient index, using probabilistic tools to identify and select the correct patient record, adopting technology and educating stakeholders, and monitoring performance, duplicate records continue to be a major problem for healthcare.

Beth Haenke Just, MBA, RHIA, FAHIMA, a recognized patient matching industry expert is founder and CEO of Just Associates, Inc. (JA) a healthcare data integration consulting firm focused on identity data integrity, data migration and health information exchange. As an HIM and IT professional, she has experience working with all types and sizes of healthcare organizations. Ms. Just served on the AHIMA Board of Directors from 2009 – 2011 and was Chair of both the Finance and Industry Committees. She is currently on the Board of Trustees for the College of Saint Scholastica. She serves on CORHIO’s technical advisory committee and volunteers her expertise to the Colorado Telehealth Network (CTN). She has also served as President of CHIMA and on numerous CHIMA and AHIMA committees and work groups. David Marc is an assistant professor and the health informatics graduate program director at the College of St. Scholastica. Dr. Marc has a master’s degree in biological sciences and a PhD in health informatics from the University of Minnesota and is a certified health data analyst (CHDA). Currently, Dr. Marc serves as the chair of the AHIMA foundation research network, a current presenter for the AHIMA faculty development regional meetings, an instructor for the CHDA exam prep workshops, serves on the Minnesota HIMSS board as chair of student affairs, and is the chair for the AMIA education taskforce.
