The presentation will describe a case management approach to assisting young adults with dual diagnoses make a successful transition from long term educational placement to adult community based services. Part one will explore techniques for creating a service plan which addresses developmental issues, vocational and psychological skills needed for adult living. Part two will focus on clinical techniques used by case managers to assist individuals to understand, accept, and manage psychiatric symptoms. Case presentations will illustrate the application of the techniques and recommendations described during the sessions.
