Advance registration must be recieved by 5pm eastern daylight savings time October 17, 2014. All participants, including invited speakers, are required to register and pay the registration fee.

Group Registration
If you are registering multiple people for the meeting, please fill out a registration form for each registrant and email, mail or fax the forms to the SIMB office. If you prefer to register them online, please contact registrar@simbhq.org before proceeding with online registration.

Cancellation requests fro refunds must be submitted in writing to registrar@simbhq.org before 5pm eastern daylight savings time, October 9, 2014. Refunds will be issues less a $75 USD administrative fee. No refunds will be made after this date. Substitutions are permitted.

Wire Transfer Payments - Please note that a wire transfer fee of $40 needs to be added to your registration total. Contact registrar@simbhq.org for bank information. If an invoice is needed with the wire transfer fee included with your registration total, please note this in your bank information request. If you are registering online please select the "Bill Me" option (we will add the fee). If you are submitting a registration form, indicate that you will be paying by wire transfer.
