The lateral wall sinus approach has been demonstrated to be a predictable, yet invasive, procedure to augment the posterior maxilla. We can now augment the posterior maxilla utilizing rotary instrumentation and hydraulic pressure to atraumatically infracture the sinus floor. Gone are the days of "bang, bang, bang" with a mallet. We can now perform the subantral bone graft procedure with ease and predictability. The surgical rationale and management will be discussed.
